HOW TO APPLY FOR WIC
Apply for WIC in 3 easy steps!

Step 1: Find a WIC Clinic

Step 2: Make an appointment to apply for WIC.

Friendly WIC staff will help you schedule an appointment and will answer your questions about WIC.

Step 3: Bring identification, proof of residency, and proof of income to your WIC appointment.

  • Examples of proof of identification include Driver’s License or state issued identification card, birth certificate, immunization record, tribal enrollment card, social security card, or your AHCCCS card.
  • Examples of proof of residency include Driver’s License or State issued ID Card, bank statement, utility bill, payroll stub, rent/lease agreement, or your tribal enrollment card.
  • You may be eligible if your income is in the range listed on the table according to the number of people living at your house. REMEMBER you can count your unborn child as a family member.
  • Examples of proof of income include Current pay stub including the pay time frame, most recent years income tax return, letter from employer, or your Tribal per capita check stub.
  • You are automatically income eligible for WIC services if you currently receive TANF, Food Stamps or Medicaid (AHCCCS).
  • Indian Families living on reservation lands do not need to show proof of income.
  • WIC staff will talk with you and fill out the information needed. You do not have to fill out a lot of paperwork.
  • Eligible clients get WIC checks for food on the day of their appointment. No waiting!

Who can apply?

  • Pregnant Women
  • Women who are breastfeeding a baby under one year of age
  • Women who have had a baby in the past six months
  • Parents, step-parents, guardians, and foster parents of infants and children under age five can apply for their children

Are services free?

Yes! All services are free to those who are eligible.
